What documents do I need to submit?

• Fully completed claim form
• In-Patient Discharge Summary Form / Medical reports
• Medical bills and final hospital bills for accident claims
• Final hospital bills for hospitalization claims

We will request for the Medical Certification form to be completed by the attending doctor if we need more information.

Personal Accident Claims

What documents do i need to submit?

• Medical bills and final hospital bills for injury claims.
• For death claims please provide police report, death certificate and coroner’s report.

Repatriation Expenses Claims

What documents do i need to submit?

• Fully completed claim form
• Doctor’s Certification on the domestic maid’s condition and/or inability to continue her current occupation
• Death certificate
• Bills and supporting documents for funeral arrangements
• Bills and supporting documents for repatriation expenses

Miscellaneous Section Claims

For Wage Compensation and Levy Reimbursement claims, benefits will be computed based on the number of days the domestic maid is hospitalized. Forms to submit:

• Fully completed claim form
• In-patient discharge summary and final invoice showing number of days of in-patient stay
• For Recuperation Benefits claims, benefits are payable to the domestic maid and will be computed based on the number of days the domestic maid is hospitalized.
• Ensure that the domestic maid has a local bank account
• If payment is to be made to you, an authorization letter signed by the domestic maid must be provided.
